Te Wakahuia is a Primary Health Care Team (PHCT) that actively promotes shared leadership, continuous quality improvement in primary health care practice and service innovation. The overall aim of the Highbury PHCT is to strive for excellence in the delivery of Whānau Ora Services to our Communities and improving access to community health and social services, co-ordination and continuity of care and reduce inequities for whānau through a collaborative approach to service delivery ensuring a more effective and efficient use of community health resources.
The Highbury PHCT comprises a number of highly skilled professionals from various disciplines, including nursing, community health workers, counsellors, social workers, health promoters and kaiwhakaaraara, who work efficiently and effectively to assist whānau to realise their aspirations. Te Wakahuia is not a General Practice Service.
